Zusammenfassung
Einleitung
In der operativen Therapie des bilateralen vesikoureterorenalen Refluxes (VUR) im Kindesalter ist die cross-trigonale Ureterozystoneostomie (UCN) nach Cohen eine populäre aber ebenso umstrittene Option. Wir präsentieren in einer retrospektiven Analyse eigene Erfahrungen mit dieser Methode.
Patienten und Methode
41 Kinder (26 Mädchen/15 Jungen) mit bilateralem VUR [92 renale Einheiten (RE)] wurden zwischen 1990 und 2005 nach Cohen operiert. Das Durchschnittsalter lag bei 4,5 (0,5–12,0) Jahren. 8 Patienten hatten einen Ureter duplex (6-mal unilateral, 2-mal bilateral); 12/41 Patienten wurden ohne perioperative Harnleiterschienung operiert; 7 Patienten waren wegen VUR voroperiert. Der Erfolg der Operation wurde definiert als fehlender Nachweis eines Rezidivs, einer signifikanten Harntransportstörung oder Blasenentleerungsstörung im Langzeitverlauf.
Ergebnisse
Der durchschnittliche Follow-up betrug 7,8 (0,5–15) Jahre. Bei 8 Patienten (19,5%) fanden sich 13 Komplikationen. Ein Patient (2%) hatte eine intraoperative Verletzung der Dünndarmserosa, 6 Patienten (14,6%) hatten fieberhafte Harnweginfekte (HWI). 4 Patienten (9,8%) zeigten eine passagere Harntransportstörung (HTS), 3 dieser Patienten benötigten eine perkutane Nephrostomie, davon hatten 2 Patienten keine perioperative Harnleiterschienung. Rezidive des VUR hatten 2 Patienten (4,8%) bzw. 2/92 RE (2,2%). Komplikationen waren häufiger bei hochgradigem VUR, Ureter duplex oder ungeschienter UCN. Eine Voroperation hatte keinen Einfluss auf die Komplikationsrate. Postoperative Blasenentleerungsstörungen wurden nicht dokumentiert.
Schlussfolgerung
Bei 2 Rezidiven des VUR musste ein Patient reoperiert werden. Eine Reoperation aufgrund einer HTS war nicht erforderlich. Die operative Erfolgsrate lag, bezogen auf 92 RE, bei 97,8%. Die Durchführung einer perioperativen Harnleiterschienung muss in der Diskussion um die Verkürzung der stationären Liegedauern überdacht werden. Ungeschient scheint nach unseren Erfahrungen das perioperative Komplikationsrisiko insbesondere bei hochgradigem VUR und Ureter duplex erhöht zu sein.
Abstract
Introduction
In bilateral VUR, Cohen cross-trigonal ureteric reimplantation is a popular but also controversial surgical approach. We present our own experience in a retrospective analysis.
Patients and methods
Between 1990 and 2005, 41 children (26 girls, 15 boys) with bilateral reflux [92 renal units (RU)] underwent ureteric reimplantation. The mean age was 4.5 (0.3–12) years. Eight patients had ureteral duplication (six unilateral, two bilateral); 12 of 41 patients had no intraoperative ureteral stenting. Seven patients had prior surgery for VUR. A successful result was defined as absence of VUR, significant UVJ obstruction, or voiding dysfunction throughout the follow-up.
Results
The mean follow-up was 7.8 (0.5–15) years. Eight patients (19.5%) had 13 complications. One patient had an intraoperative small bowel lesion (2%). Six patients (14.6%) had UTI. Four patients (9.8%) showed transient UVJ obstruction. Three required a temporary percutaneous nephrostomy. Two of these patients had no intraoperative ureteral stenting. Recurrence of VUR was found in 2 patients (4.8%) and 2/92 RU (2.2%), respectively. Complications were more frequent in high-grade VUR, ureter duplex, or unstented ureteral reimplantation. Prior surgery for VUR did not influence the postoperative outcome. Postoperative voiding disorders were not observed.
Conclusions
Two unilateral recurrences of VUR were observed, requiring a reoperation in one patient. A reoperation for UVJ obstruction was not necessary. Related to 92 RU the surgical success rate was 97.8%. Intraoperative ureteral stenting has to be considered with respect to the current discussion of shortening inpatient procedures. In our experience, the perioperative risk was elevated in patients with high-grade VUR or ureteral duplication.
