Leber's disease in the Netherlands

Summary

An investigation was made into the occurrence of Leber's disease in the Netherlands.

300 male and 52 female Leber patients were found. 202 patients were examined, a few at home, the majority in one of our large hospitals, the staffs of all of which assisted in this investigation.

For the purpose of this investigation, a fixation ring for use in the Goldmann perimeter was devised, by means of which reliable visual field analysis was possible in spite of central scotomata.

Peripheral abnormalities of the visual field were seldom found. Central abnormalities were almost always present. These central scotomata became manifest in 42.8 % of the cases within 10 minutes.

Observations were made which make it appear probable that the nervous connection with the area of the scotoma is not obliterated but only altered.

It was found that, in the case of 25 % of the patients, the visual acuity in at least one eye improved spontaneously to 0.4, but that this visual acuity could not always be used in the normal way. A phenomenon was observed which I have called “delayed vision”: the maximum visual acuity was often only attained after some minutes of peering. As a result of this, these patients read much more slowly than persons with a similar visual acuity as the result of other conditions.

There is a tendency towards a given age of onset within a family, this is not appreciably different for males and females.

In a number of cases disturbances of dark adaptation were found.

The combination with paraplegias was relatively very frequently found.

The only result obtained by neurosurgical exploration was found to be an earlier onset of improvement.

45.2 % of patients showed spontaneous improvement in at least one eye.

82 % of the patients were able to make their own living.

It appears that atypical, and possibly also subclinical, cases can occur.

All the pedigrees were compiled from particulars supplied and controlled by the registration service, supplemented by particulars from old baptismal and marriage registers and legal archives.

The patients were found to belong to 46 pedigrees. 19 pedigrees were found to be connected with other pedigrees by relationship in the female line, thus 27 more extensive pedigrees were formed. One of these was formed from no less than 10 smaller pedigrees. We conclude from this, that the disease does not die out rapidly, and that mutations only occur very occasionally.

We deduced from the results of the genetic investigation that ± 50 % of the sons of carriers, and 8.5 % to 15 % of the daughters of carriers are affected.

No affected persons were found among the descendants of male Leber patients.

It was calculated that among the children of affected women ± 50 % of the sons and 24.2 % to 35.2 % of the daughters have the predisposition.

The percentage of carriers was calculated by 2 different methods, the results were ± 70 % and ± 96 %.

I did not manage to find an explanation of the mode of inheritance: the high percentage of carriers found argues against straightforward chromosomal transmission, and the percentage of persons affected against straightforward cytoplasmic transmission.

There is no indication that Leber's disease will die out, unless strict eugenic measures are taken.

Nous avons fait des recherches au sujet de la maladie de Leber en Hollande.

Nous avons trouvé 300 hommes et 52 femmes atteints de cette maladie.

202 malades furent examinés, quelques-uns chez eux, presque tous dans nos grandes cliniques qui, toutes, nous ont assisté dans nos recherches.

En vue de cette enquête, j'ai construit un anneau fixateur dans le périmètre de Goldmann, permettant des analyses sûres du champ visuel, malgré les scotomes centraux.

Les déficits périphériques du champ visuel étaient rares; les déficits centraux étaient la règle.

Ces déficits centraux se manifestaient en moins de dix minutes chez 42,8 % des malades.

Il résulte de nos observations que, très probablement, la connection nerveuse avec la région du scotome est seulement alterée par l'affection, et non pas abolic.

Nous avons constaté que chez 25 % des malades l'acuité visuelle d'un des yeux au moins s'améliorait spontanément jusqu'à 0,4, mais que souvent le malade ne pouvait pas se servir de façon normale de cette acuité.

Nous avons observé un phénomène que j'ai appelé “acuité visuelle retardée”. Souvent l'acuité visuelle maxima n'était atteinte qu'après avoir regardé fixement pendant plusieurs minutes. Il en résulte que les malades lisent bien moins vite que les personnes à acuité semblable, provenant d'autres causes.

L'age de la première atteinte se trouvait être déterminé par la famille; il n'y a pas de différence sensible entre hommes et femmes.

Dans un certain nombre de cas, nous avons observé des troubles de l'adaptation à l'obscurité.

Relativement très fréquente était la combinaison avec la paraplégie.

L'exploration neuro-chirurgicale avait pour seul résultat l'accélération d'une amélioration éventuelle.

Chez 45,2 % des malades, nous avons pu constater une amélioration spontanée pour un des yeux au moins.

82 % des malades pouvaient se suffire.

Des cas atypiques, peut-être aussi des cas subcliniques, peuvent se produire.

Toutes les généalogies sont basées sur les données fournies par l'état civil, complétées par celles provenant des vieux registres de baptême et de mariage et des archives judiciaires.

Les malades appartiennent à 46 généalogies. Par parenté dans la ligne féminine 19 généalogies se combinaient avec d'autres, ce qui a donné 27 généalogies plus étendues. Une de ces généalogies se compose de 10 généalogies particulières.

Nous en concluons que la maladie de Leber est lente à s'éteindre et que les mutations sont extrêmement rares.

De investigations génétiques nous déduisons que ± 50 % des fils, et 8,5 à 15 % des filles de porteuses de maladie étaient atteints.

Parmi les descendants de malades mâles nous n'avons pas trouvé de personnes atteintes de la maladie de Leber.

Parmi les enfants de femmes atteintes de la maladie il y avait, selon mes calculs, ± 50 % de fils atteints et 24,2 % à 35,2 % de filles.

Le pourcentage de porteuses de la maladie a été calculé selon deux méthodes différentes, les résultats étaient ± 70 % et ± 96 %.

Je n'ai pas réussi à trouver une explication de la transmission héhéditaire: le grand pourcentage de porteuses de la maladie forme un argument contre une transmission chromosomale directe, tandis que le pourcentage des personnes atteintes forme un argument contre la transmission cytoplasmale directe.

Il paraît peu probable que la maladie de Leber s'éteigne bientôt, à moins que l'on applique des mesures eugéniques très rigoureuses.

Riassunto

E stato fatto una investigazione sulla occorrenza della malattia di Leber in Olanda.

Sono stati scoperti 300 pazienti maschili e 52 feminili. 202 pazienti erano esaminati, alcuni a casa propria, ma la maggior' parte in una delle nostre grande cliniche, che hanno tutti cooperato a questa investigazione.

Per questa investigazione abbiamo construito un anello di fissazione nel “perimetro di Goldman”, con quale divento possibile un' analisi fidata del campo visivo, nonostante gli scotomi centrali.

Gli defici periferici del campo visivo risultavano pocco, gli defici centrali quasi sempre. Questi scotomi centrali si sono manifestati da 42,8 % degli pazienti prima di 10 minuti.

Delle investigazioni dimostrano che, probabilmente, la connessione nervosa con la regione del scotoma non e obliterata, ma solamente alterata. Abbiamo constato che a 25 % degli pazienti l'acuità visiva di almeno un occhio si amegliorava spontanamente fino a 0,4, ma che sovente il amalatto non si poteva servire della maniera normale di quell'acuità.

Abbiamo osservato un fenomeno che abbiamo chiamato: visione ritardata. Sovente l'acuità visiva massima e stato ottenuto soltanto doppo aver' fissato di minuti. Risulta che il malatto legeva piu lentamente che le persone con un'acuità simile provenendo di altri causi.

L'età iniziale e trovato d'essere determinato nella famiglia. Non cera differenza significativo tra maschii e femine.

In certi casi abbiamo trovato un adattamento disturbato all'oscurità. Relativamente la combinazione con le paraplegie era molto frequente.

L'esplorazione neurochirurgica aveva il solo risultato che un ameglioramento eventuale era accelerato.

A 45,2 % degli amalatti abbiamo constato un ameglioramento a almeno un occhio.

82 % degli pazienti si posseno mantere da se stessi.

Si occoreno degli casi atipici, forse anche subclinici.

Tutti gli geneologi sono basati sulle informazioni fornite e controlate del stato civile, suplementate con degli dati del registro battesimale e di matrimonio e del archivo giustiziare.

Gli pazienti hanno appartenuto a 46 geneologi. Per parentela per parte feminile, 19 geneologi si combinavano con degli altri, in modo que risultavano in 27 geneologi piu estensivi. Uno di questi geneologi si composava di non meno di 10 geneologi separati. Si conclude che la malattia non si cessa fra pocco e che gli mutazioni sono estremamente rari.

Abbiamo calcolato della investigazione genetica che ± 50 % degli figli, e di 8,2 % a 15 % delle figlie delle portatrice hanno la malattia.

Sotto gli descendenti degli amalatti maschili non si a trovato dei pazienti. Abbiamo calcolato che sotto gli figlii delle donne amalatte ± 50 % degli figli e 24,2 % a 35,2 % delle figlie hanno la predisposizione per la malattia.

Il percento delle portatrice si calcolava in due maniere differenti, risultando in ± 70 % e ± 96 %.

Non siamo riusciti a trovare una esplicazione per la maniera di trasmissione ereditaria: il percento alto delle portatrice argumenta contro un' eredità diretta cromosomale, il percento degli amalatti contro un' eredità diretta citoplasmale.

Non pare probabile che la malattia di Leber si cassi fra pocco, a meno che non si applica misure eugeniche severe.

Zusammenfassung

Bei einer Untersuchung nach dem Vorkommen der Leber'schen Krankheit in den Niederlanden wurden 300 männliche und 52 weibliche Patienten gefunden. 202 Patienten wurden untersucht; einige in ihren Wohnungen, die meisten in Krankenhäusern, die alle dieser Unternehmung ihre Mitarbeit verliehen.

Für diese Untersuchung wurde von dem Autor ein Fixationsring zur Benutzung im Goldmann-Perimeter entworfen, durch den trotz zentraler Skotome eine zuverlässige Gesichtsfeldanalyse ermöglicht wurde. Es zeigte sich, dass periphere Gesichtsdefekte wenig, zentrale dagegen beinahe immer vorkommen. Diese Zentralskotome manifestierten sich bei 42,8 % der Patienten innerhalb von 10 Minuten.

Die Beobachtungen machen es wahrscheinlich, dass die Nervenverbindung mit dem Skotom durch die Krankheit nicht unterbrochen, sondern nur verändert wird. Es zeigte sich, dass die Sehschärfe bei 25 % der Patienten mindestens an einem Auge anstieg auf 0,4, dass aber diese Sehschärfe nicht immer normal von den Patienten ausgenutzt werden kann.

Das von dem Autor “verzögerte Sehschärfe” (delayed visual acuity) genannte Phänomen besteht darin, dass die maximale Sehschärfe oft erst nach minutenlangem Starren erreicht wird. Hierdurch ist z.B. die Leseschnellheit viel geringer als bei Personen mit gleicher, aber durch andere Ursachen hervorgerufener Sehschärfe.

Der Autor fand das Anfangsalter familiär bedingt, jedoch ohne signifikanten Unterschied zwischen Männern und Frauen.

Einige Male wurde eine gestörte Dunkeladaptation gefunden.

Die Kombination mit Paraplegien wurde relativ sehr frequent angetroffen.

Neurochirurgische Exploration hatte als einzigen Erfolg den schnelleren Anfang der eventuellen Besserung.

45,2 % der Patienten verbesserten sich spontan, wenigstens an einem Auge.

82 % der Patienten konnten selber für ihren Unterhalt sorgen.

Es zeigte sich, dass atypische und möglich auch subklinische Fälle vorkommen.

Alle Unterlagen für die Stammbäume wurden von dem Zivilstand geliefert, kontrolliert und vervollständigt mit Daten aus alten Tauf- und Heiratsurkunden und Gerichtsarchiven.

Es zeigte sich, dass die Patienten 46 Stammbäumen entstammten.

Durch Verwandtschaft in weiblicher Linie konnten 19 Stammbäume mit anderen vereinigt werden, wodurch 27 grössere Stammbäume entstanden. Hieraus konnte geschlossen werden, dass die Krankheit nicht schnell ausstirbt und Mutation nur sehr selten vorkommt.

Bei der Hereditätsuntersuchung berechnete der Autor, dass ± 50 % der Söhne von Konduktrizen und von 8,5 % bis 15 % der Töchter von Konduktrizen die Veranlagung für die Krankheit besitzen.

Unter den Nachkommen von männlichen Patienten werden keine kranken Personen gefunden. Von den Kindern kranker Frauen haben nach der Berechnung des Autors ± 50 % der Söhne und von 24,2 % bis 35,2 % der Tächter die Veranlagung, der Krankheit anheimzufallen.

Der Hundertsatz der Konduktrizen wurde auf zweierlei Weise berechnet; die gefundenen Werte betrugen ± 70 % und ± 96 %.

Es ist mir nicht gelungen, eine Erklärung der Übertragungsweise zu finden: der grosse Hundertsatz an Konduktrizen spricht gegen normale chromosomale Heredität und der grosse Hundertsatz kranker Personen gegen normale zytoplasmale Übertragung.

Es scheint ausgeschlossen, dass die Leber'sche Krankheit bald aussterben wird, ausser im Falle strenger eugenetischer Massnahmen.
