Adjusted ORs are presented from a multivariable regression model including all variables in the table. Patients with missing data in any of the variables are excluded (n=2728), resulting in inclusion of 5200 patients in the multivariable model. Abbreviated results (highest and lowest ORs) are presented for the Cancer Alliance (full results available in online supplemental table 4). P values <0.05 are highlighted in bold. ORs and p values are presented to three and two significant figures, respectively.
