  • Proband diagnosed with PrCa below the age of 50 years

  • Ashkenazi Jewish Heritage with PrCa at any age

  • Proband diagnosed with metastatic PrCa below the age of 60 years

  • Proband diagnosed with PrCa with a family history of cancer where estimated likelihood of identifying a pathogenic variant in the relevant target genes is at least 10% (using the CanRisk tool18)

  • Genetic testing may occasionally be appropriate outside these criteria following discussion at a specialist multidisciplinary team (MDT) meeting with a cancer geneticist present
