Case

A 33-year-old white primigravida underwent in vitro fertilization with a donor egg. Her medical history was significant for Turner syndrome and hypertension. The patient had been admitted to a local hospital at 24 weeks’ gestation complaining of nausea, vomiting, and worsening epigastric pain over a 2-day period. She began intravenous magnesium sulfate for presumed preeclampsia and subsequently developed acute pulmonary edema. She was transferred to New York Hospital for further care. Her pulse

Comment

Recent advances in reproductive technology have enabled many women to conceive. These include women with serious medical and genetic conditions. We are becoming aware only now of the potential problems of pregnancy for some of these women. Preconceptual counseling and medical evaluation should be performed before conception in these cases.

    Women with TS are predisposed for a wide range of cardiovascular diseases such as hypertension, bicuspid aortic valve and aortic aneurisms and dissections [10,69,70].     Finally, the present case demonstrates the strength of the desire of these patients to conceive, despite the known risks. Several deaths with aortic dissection have been reported in Turner syndrome patients, who became pregnant through oocyte donation (43–47). Karnis et al. estimated that at least four deaths occurred in approximately 200 donor oocyte pregnancies in women with Turner syndrome.
